With this request, I understand the conditions used to determine my eligibility for a book voucher are the following: All admissions requirements are complete All financial aid paperwork is completed The Financial Aid Office has received a signed bill validating a credit (-) for the current term of enrollment All academic requirements are upheld after the first term of enrollment

I understand this form will be sent to my Financial Aid Counselor, who must approve this request and may contact me if additional information is necessary.
